Why doesn't ABPA show up on an X-ray?

Dr. C
ABPA, otherwise known as allergic bronchopulmonary aspergillosis, is a disease that results in severe asthma-like symptoms. The fungus, aspergillus, is found throughout the environment and can colonize the airways. In many cases, no symptoms occur. However, in ABPA, the body's immune system reacts against its presence. X-rays cannot detect the fungus itself. However, there are certain conditions that may predispose someone to aspergillus colonization. In particular, structural airways disease, such as bronchiectasis, COPD, asthma, and other inherited disorders may put someone at higher risk for ABPA. Some of these disorders may be detected by X-ray. In essence, ABPA cannot be seen by X-rays, but the predisposing disease may.

Just found a website with the following description: A wide variety of chest radiographic changes are known to be associated with ABPA. These changes include normal chest radiography, hyperinflation, various infiltrate patterns, consolidation, parallel lines and ring shadows, nodules, avascular areas, “honey combing”, “tooth paste” shadows, “gloved finger”, band shadows and tramline shadows, changes like fibrosing alveolitis, lobar shrinkage and atelectasis as well as pseudohilar adenopathy and pleural thickening. Although the chest radiographic appearance is commonly abnormal in patients with ABPA, it is not invariably so, nor does a normal appearance exclude the diagnosis of ABPA. Hope this helps =]
