Ear infection causing excruciating pain on entire right side of head?
About 10 days ago I started having pains in my ear which I ignored. There would be intermittent jaw pain as well so I wasn't sure where the pain was originating. 5 days later I was diagnosed with an acute middle ear infection and a sinus infection I wasn't even aware of having. I've been taking the antibiotics prescribed since then. I thought it was clearing up, but tonight the pain returned in full force. I've also been nauseated, lethargic, and today I started to have dizzy spells. I AM having a few unpleasant side effects from the Keflex, but nothing out of the ordinary.

I have excruciating pain radiating up and down the entire right side of my head/face. Right above my eye, my temple, above my ear, my cheek, my jaw, my neck, and even my gums are killing me right now. I also have some stiffness in my neck and the pain feels like it's radiating up my spine into my head back there as well. I've been taking ibuprofen, tried distracting myself, and even laying down but the only way I can get the pain to ease off at all is if I sit straight up with my legs hanging down. If I go to the couch and relax back with my legs up the pain amplifies again. It even hurts just to touch any of those areas and forget about laying down on that side.

Is this normal for an ear infection or do I need to go to the ER? I hate going to the emergency room when it's not an emergency, but I have no idea what's going on here.

All the pains you listed are caused by tight neck muscles. When your neck muscles are tight they restrict everything going in and out of your head, including the blood When there is a reduced blood flow to the brain you will end up with a lowered oxygen level there as well. You will know if you have a lowered oxygen level by feeling very tired. If the level drops more you will get dizzy and more you will pass out. When your neck muscles are tight and restricting what is going out of your head you will end up with a back up of wastes that are supposed to leave the area. When they back up they fill your sinuses to give you trouble with them. Your neck muscles go to the top of your head passing just inside of your ears. When the muscles are tight they can press into the nerves for your ears to give you pains in them. When the muscles are tight at the top they pull on other muscles that go around your head and when they get tight they pull on the areas where they are attached to the head. When the muscles that are around your brain tighten up they give you your headache as well. To get rid of these pains you have to free up your neck muscles and here's how to do that:
Put your hands alongside your head so your thumbs are on the front of the muscle under your ear and your fingers are on the back of the muscle behind your neck. Squeeze your thumb and fingers together and hold. Relax your body. When your fingers and thumb touch, about two minutes, slowly lower your head as far as you can, release the pressure but hold your neck lowered for another 30 seconds.
If you still have pain in front of your ear where the jaw is it's the muscles for your jaw giving that pain and to get rid of it you have to free up the muscles in your jaw and here's how to do that:
Jaw muscles, TMJ:
Put your fingers on your head so your thumbs are behind your ears. Press into the back of your jaw next to the skull and hold that pressure. After 30 seconds slowly raise your head until your neck is fully extended then open your mouth as far as you can. Release the pressure but hold your head and mouth like that for another 30 seconds.
You also mentioned back pain and if that is still around after doing the other muscles here's how to free them back up to get rid of those pains, just so you are covered on all your listed pains:
(do from a sitting position)
Place your left hand on your left leg next to your body. Place your right hand over your left shoulder, fingers over the back and the palm in the front and firmly pull down on them and hold. After 30 seconds slowly lower your body forward and to the outside of your left leg, keeping your left arm fairly straight as you do. When you reach your lap remain there for another 10 seconds, release the pressure but rest there for another 30 seconds. Then reverse your hand positions and do your right side. for all- for best results relax your body first by taking a deep breath and exhaling then remain this relaxed.

Yeah you need to go the the ER. It is pretty obvious the infection was not cleared up by the antibiotics thay gave you, and the infection may have spread to your inner ear, since you are getting dizzy.

You also need to get someone to drive you, or take an ambulance, since driving yourself would be dangerous with the nausea and dizziness.
